EverTrue East Staff Reimbursement for Ongoing GLP-1 Medication

If an EverTrue East employee, or one of their dependents, covered under both a Diakon Medical Plan in 2024 and a EverTrue Medical Plan in 2025, was prescribed GLP-1 medications for weight loss purposes (i.e. not due to a diabetes-related condition) in 2024 and has chosen to continue adherence to that prescription medication in 2025, below is the process for requesting and receiving reimbursement for your out-of-pocket expenses.

1.       Employee (or dependent) must remain actively enrolled in a EverTrue medical plan to be eligible for this reimbursement.  Employee must complete and sign the Agreement prior to the first reimbursement payment being made that agrees to repay EverTrue any reimbursement amounts paid to them if they leave EverTrue within 6-months of the reimbursement payment date.  Accompanying this Agreement must be a 2024 EOB (explanation of benefits) providing confirmation of a GLP-1 drug being prescribed and taken during the 2024 plan year.  This Agreement and supporting documentation for 2024 must be submitted to the EverTrue Director of Benefits & Compensation at Denis.Thien@EverTrueLiving.org prior to any requests for reimbursement being submitted.

2.       Once approval has been received to participate in this 2025 reimbursement program, requests for reimbursement to cover out of pocket expenses related to continued utilization of the GLP-1 drug must be submitted to the EverTrue Director of Benefits & Compensation via email (Denis.Thien@EverTrueLiving.org) and specifically state the date of the employee’s expense and the total dollar amount the employee paid out of pocket.  Accompanying these details must be formal documentation verifying the out-of-pocket expenses paid by the employee or their dependent.

3.       Following the submission of the information in step #2, a check request will be initiated through the EverTrue Accounting department and reimbursement paid directly to the employee.  The maximum reimbursement an employee may be eligible for is $600.00 per month.
